- Description
- Certificate presented to Dorothy Johnson "in recognition of her distinguished services and personal contributions of time and effort" to the Order of the Eastern Star and to the Eureka Chapter No.11.
- The certificate is printed in black ink on cream colored paper, with names added in typewritten black ink and signatures completed by hand in black ink. There is a gold sticker with red, white and blue striped ribbon attached at the bottom left corner. At center top of the certificate is the Eastern Star emblem which consists of a five pointed star with a pentagon at the center. It is signed by Florence L. Maultsby, George E. Johnson, and Hazel Addington, Secertary. At botton right, stamped in blue ink is [OCT 15 1986].
