Description
A blank, unused Certificate of Examination card printed by the Virginia State Health Department and used by the midwife Amanda Carey Carter.
The small pink card has pre-printed black text with spaces to be completed by hand. At the top [CERTIFICATE OF EXAMINATION]. The rest of the card reads [Name (space) / Has been examined during this pregnancy and may be deli - / vered by a midwife if no complications develop. / Blood tested (space) / Date / Date (space) Signed (space) / M.D. / Va. State Health Dept. / MCH-214 / (OVER)].
The back of the card is printed with instructions for both doctor and patient: [Doctor: Present this card to the patient after 8th month of / pregnancy. / Patient: Give this card to the midwife when she is called for / delivery.]
