A paper "Midwife's Certificate of Retirement" issued by the Virginia State Department of Health to Susie Carey. The certificate has an intricate orange scalloped border and is printed in black ink and completed by hand also in black ink. The center text reads [THIS CERTIFICATE IS PRESENTED TO / SUSIE CARY OF FARMVILLE / PRINCE EDWARD COUNTY WHO HAS BEEN A MIDWIFE / FOR 33 YEARS. SHE HAS RETURNED HER PERMIT ON / THIS 16TH DAY OF JUNE 1952 AND WILL NOT / ATTEND ANY OTHER CASES.]. The certificate is signed at lower right by the state registrar of vital statistics and the county health officer. A faintly visible watermark reads "HAZEL BOND".

Originally framed with the retirement certificate is a newspaper clipping of a memorial poem for Susie Carey written by her daughter, Amanda Carter. The headline reads [In Memorian] and the text begins "In memory of my darling mother, Mrs. Susie Carey, who passed away 13 years ago, June 9, 1953." At bottom right is "Loving daughter, Amanda Carter, 6-10-1p".
