- Description
- A printed form on a single sheet of yellowed paper. At the top is printed [INMATE TELEPHONE LIST] in bold and underlined text. Beneath this are spaces for "Name" and "DOC No," which have been completed by hand with "Herman Wallace" and "76759." Along the left side is a numbered table with telephone numbers entered by hand. There are various notes made to the list: "Out" and/or "X" has been added to several lines and some telephone numbers have been crossed out. Surrounding the numbered list and on both sides of the sheet are numerous additional notes, names, and telephone numbers in black and red inks. The paper has been folded into twelve sections.
